NEXPLANON® Indications

NEXPLANON® is a subdermal implant that releases the hormone etonogestrel, offering a long-term contraceptive solution.

By releasing etonogestrel, it effectively prevents ovulation, alters cervical mucus to prevent sperm entry, and modifies the uterine lining to prevent implantation of a fertilized egg.

NEXPLANON® is ideal for patients who want an extended period of contraception without the need for daily attention. 

After insertion, the NEXPLANON® implant begins a steady release of etonogestrel into the bloodstream. 

The pharmacokinetic profile of NEXPLANON® is characterized by an initial peak, followed by a plateau, and eventually a decline in hormone levels over its three-year duration of action. 

This steady release ensures a consistent plasma concentration that is sufficient to maintain its effects without the need for daily or monthly dosing.

NEXPLANON® Administration Technique

To ensure that your patient gets immediate protection, try to insert the implant within the first five days of the menstrual cycle. If timing doesn’t align, advise your patient to use alternative contraception for the next seven days.

The inner side of the upper arm offers the most convenient place for the implant. After cleaning the area, apply a local anesthetic to ensure the procedure is as comfortable as possible for your patient.

Use the preloaded NEXPLANON® applicator to place the implant under the skin. Create a skin ‘tent’ to accurately place the implant in the subdermal layer. This way you ensure that the implant remains discreet and secure.

Wrap up by applying a pressurized bandage to minimize bruising of the insertion site. Also, hand your patient a reminder card for the implant’s removal or replacement in three years. 

NEXPLANON® Side Effects

NEXPLANON® is a very effective method for preventing unwanted pregnancies, but like all contraceptives, it can have some side effects. Let’s take a closer look at them.

Common Side Effects

• Changes in Menstrual Bleeding Patterns: Patients may experience irregular periods, lighter or heavier menstrual bleeding, or even complete loss of the period. (amenorrhea). It’s important to counsel patients that these changes are normal and usually not indicative of any serious condition.
• Mood Changes: Some patients may report mood swings, depression, or anxiety. 
• Weight Gain: Patients may notice weight gain while using NEXPLANON®. Encourage a balanced diet and regular exercise, and monitor their weight and overall health regularly.
• Acne: Skin changes, such as acne, might develop or worsen. Discuss skincare routines and possible treatments if acne becomes a concern.

Less Common Side Effects

• Breast Tenderness or Pain: Some patients report tenderness or pain in their breasts. This is usually temporary and can be managed with over-the-counter pain relievers.
• Ovarian Cysts: Etonogestrel can occasionally lead to the formation of ovarian cysts. These are typically benign and resolve on their own but can cause discomfort or pain if they rupture.
• Liver Disease: Monitor for signs of liver disease, such as jaundice, as etonogestrel can affect liver function.

IUDs come in two main types: hormonal (releasing levonorgestrel) and non-hormonal (copper). Hormonal IUDs share some side effects with NEXPLANON®, such as changes in menstrual bleeding patterns and ovarian cysts, but also include device-specific risks like uterine perforation or expulsion. 

Copper IUDs don’t have hormonal side effects but can increase menstrual bleeding and cramping.

Injectable Contraceptives

Progestin-only injectables are administered every three months. Its side effects include menstrual changes, weight gain, and bone density reduction over time.
